  • Out of Hours - NHS 111 - This service is for when you need medical help fast but it is not a 999 emergency.

How to Register with the Practice

We welcome new patients. If you wish to register with Lyngford Park Surgery please come into the surgery and pick up a Registration Pack.

Proof of ID

Please bring ID with you if you can, however you will not be refused registration or appointments because you do not have a proof of address or personal identification at hand. This also applies if you are an asylum seeker, refugee, a homeless patient or an overseas visitor. ID is not required for children under 15.

New patients need to bring their own forms and ID to the surgery.


Forms of ID should ideally include:

  • A form of Photo ID, for example a passport or a driving licence
  • Proof of current address, such as a utility bill, bank statement, housing agreement

Our Registration Pack

You will be asked to complete a Registration Form (GMS1), a medical questionnaire, Fast Alcohol Screening Test (FAST) and the Accessible Information Standard. You can complete the forms here at the surgery on the day or complete them at home and bring them back in. Please note that we need all four completed forms before we are able to register you. New patients under 15 will be asked to complete the Registration Form only.


Immediately Necessary Treatment

If you fall ill while away from home, or if you are not registered with a GP but need to see one and are in the Lyngford Park Surgery catchment area, you can still contact the surgery to ask for treatment. You can receive emergency treatment for 14 days. After that you will have to register as a temporary resident or permanent patient.


Temporary Residents

Consider calling NHS 111 if you urgently need medical help or advice but it's not a life-threatening situation. You can also call NHS 111 if you're not sure which NHS service you need.

Registration as a temporary resident allows you to be taken on to the practice's list for up to three months. If you are registered with a practice but are away from your home area, you can register temporarily with Lyngford Park Surgery if you are in the area and still remain a patient of your registered practice. Try to have the following information available when you attend your appointment for the first time:

  • details of your ongoing medical problems
  • details of medical problems you have suffered in the past
  • details of any medicines that you are currently taking
  • details of any allergies
  • contact details of your registered or previous practice.
